Replacement packet including Eagle Scout card, certificate, and congratulatory letter displaying new requested Eagle Scout name. Indicate on form: Name on original Eagle Scout certificate, new requested name change, birth date, Eagle Scout Award month and year, City and State and Council that Eagle Scout was awarded.
Replacement credentials are in the current year's format with the current Honorary Chair and current Chief Scout Executive signature. We apologize that we are unable to provide credentials other than the current year's format.
